Prescriptions

If you need a prescription refilled, have the name of the prescription, name and number of the pharmacy where we may phone it in, and phone number where you may be contacted with any additional questions. Please note that we will not be able to renew a prescription if it has been over one year since you have seen the doctor. Also, please note that any high-risk medications and narcotics are prohibited by law to be called in for renewal.


New Patients

For your convenience, we offer a downloadable version of our New Patient Information Form. Please print and fill out the form entirely. Present the form at the time of your visit, in addition to your insurance cards, any necessary prior authorization or referrals and a current listing of medications including dosage, please include vitamins and herbal supplements, too.

HIPAA: Notice Privacy Practices

You will be asked to sign a form that describes how your personal health information is used. The form will also disclose how you can obtain access to this information. Please review this form carefully, keeping in mind that if a person or family members name is not listed on this form, we will not be permitted to give out any information. A copy of the HIPAA Notice of Privacy Practices is displayed in a three ringed binder located in the waiting room. Our Privacy Compliance Officer will be glad to answer any of your questions regarding this matter.

Insurance

Our center accepts most insurances. We also accept patients who do not have insurance. We do not want anyone to be denied medical care because of inability to pay. If you have difficulty understanding or paying your bill, we encourage you to discuss your situation with an office staff member. We will be glad to assist you. If prior authorization, or referral, is required please contact your insurance company or your Primary Care Physician (PCP) prior to your scheduled appointment. We will do our best to help you with your insurance matters but it is the responsibility of the patient to understand his/her insurance coverage. In the event of a denial, error or non-covered service, the patient is financially responsible for all services rendered.

Many insurance companies do not pay for more than one procedure at a time. As a result of these restrictions we must implement the following guidelines; procedures are limited to one per visit, problems are limited to one or two at a time and we may not be able to perform a procedure at the same time as a visit.

Expenses not covered by insurance include cosmetic procedures, deductibles, co-insurance and copayments. Patients are responsible for their copayments and/or their deductibles at the time of service. Payments may be made by cash, check or credit card. Please bring your insurance card and driver's license to your appointment.


Lab Results & Diagnostic Testing

We understand and are sympathetic to the anxiety that waiting for test results may incur. Biopsy results normally take 7-10 days. Only abnormal results are reported to patients. Should you have an abnormal result, you will be notified by a personal letter from our physician if additional treatment is necessary. Please contact our office to schedule a follow up appointment after receiving a letter.

If your other lab works results are within normal limits, we will not notify you. We prefer that you keep your follow-up appointments so that we can review your lab result with you personally.


Surgery

All skin cancer surgeries are performed at our office under local anesthesia. If you are required to have surgery you may be scheduled for a pre-operative visit. At this visit you will be given information on your diagnosis, surgery procedures, pre-operative and post-operative instructions and any necessary paperwork will be filled out. Most importantly, any questions or concerns that you have will be answered.


Office Hours

Our office hours are by appointment only. A sincere effort will be made to have you seen without too much of a delay. While you are here you will receive our undivided care and attention. If you are unable to keep your appointment, kindly call us 24 hours in advance. Most emergency appointments are added towards the end of the day. Please ask about our extended office hours on Tuesdays for our Smithfield location.
